Fees + Billing + Insurance

My general fees are listed below and serve as an introduction to the ‘Good Faith Estimate’ federal law requiring me to inform clients of my fees prior to providing a service.

Fees include paperwork, screening measures for risk, assessment of your psychological needs and ideas for treatment. Full follow-up visits are also noted below. You will be expected to pay* during each session, unless we agree otherwise. I will provide you with periodic billing summaries and receipts to submit to insurance or health care spending accounts. Payment schedules for other professional services will be agreed upon at the time that they are requested.  Fees are dependent upon a number of factors, including customary insurance rates, insurance co-pays and deductibles and an individual’s personal ability of pay for services.  In circumstances of financial hardship, especially for college students and early career professionals, I am willing to discuss and negotiate a fee adjustment or payment installment plan.  

Current Insurance Plans in which I am an In-Network Provider:  Aetna Insurance / Aetna Student Health / Cleveland Clinic Aetna Employee Plan; Anthem Blue Cross Blue Shield / University Hospitals Employee Anthem BC Plan; Cigna and Medical Mutual of Ohio.

Late arrival for appointment: client is responsible for portions of the fee, beginning with each 15 minutes of being late; at 45 minutes, the full fee applies.

Report writing + clinical correspondence 60 Miin $250

Other professional services: responding to legal demands (e.g., depositions and trial testimony), telephone conversations lasting longer than 10 minutes, attendance at meetings with other professionals you have authorized, and the time spent performing any other professional service you request of me.

* payment options include cash, checks, credit cards (including health care spending accounts), Venmo, Zelle and PayPal*

  • a service fee of 4% will be added to cover transaction fees when using PayPal, credit + HSA cards for payment. Currently, there are no service fees applied when using Venmo or Zelle for payments.
